About the Role
Ideal Candidate
- Current NSW Drivers Licence (P1 and P2 acceptable).
- Tertiary qualifications in Nursing or Allied Health (specifically Occupational Therapy, Physiotherapy, Speech Pathology, Social Work or Dietetics) registration with AHPRA or eligibility for membership to appropriate Professional Association if required.
- Experience in the comprehensive assessment, care planning, review and monitoring practices in community care service provision.
- Experience in service provision for clients who are elderly, disabled, with chronic or complex care needs or requiring end of life care; with knowledge of the delivery and monitoring of brokered services.
