The Osteoporosis Refracture Prevention (ORP) coordinator is involved in the identification, assessment, care coordination and case management of patients attending the Hornsby Ku-rin-gai Hospital (HKH) Osteoporosis Refracture Prevention Service (HKHS ORP) Services, under the clinical oversight of a Physician. This position is also required to oversee, manage and provide the DEXA service.

What you'll be doing
The HKHS ORP Service aims to prevent re-fracture in patients aged 50 and over, post minimal trauma fracture, related to low bone density (osteoporosis). It also cares for people with osteoporosis or osteopenia who have a high risk of fracture.

The Fracture Liaison Coordinator position requires musculoskeletal clinical skills as well as knowledge and experience working with chronic care populations. Experience in the clinical management of patients with osteoporosis is desirable. Integral to the success of this position is application of the principles of chronic disease management, establishing care pathways with community health care providers, and effective care coordination.
This position also manages the DEXA service.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

  1. Must hold qualifications recognised for registration with the Physiotherapy Board of Australia. Demonstrated current clinical knowledge of Physiotherapy, a high level of contemporary intervention skills, and demonstrated senior expertise in delivery, direction and management of physiotherapy services. Must have experience as a level 2 Physiotherapist.
  2. Demonstrated experience in co-ordinating multi-disciplinary care and the capacity to work cooperatively and effectively with medical, allied health professionals and consumers.
  3. Demonstrated experience in conducting service development, quality improvement activities and professional education.
