What you'll be doing
A Nurse Practitioner (NP) is a registered nurse educated and authorised to function autonomously and collaboratively in an advanced and extended clinical role. The NP role includes the assessment and management of patients using nursing knowledge and skills and may include but is not limited to initiation of diagnostic investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP role is grounded in the nursing profession's values, knowledge, theories and practice and provides innovative and flexible health care delivery that complements other health care providers (ANMC 2006). NPs practice collaboratively as an interdependent member of the multidisciplinary health care team and provide autonomous, patient centered care.
In this role, you will specialise in Complex Wound Management, providing expert clinical leadership in the assessment, treatment and ongoing management of patients with acute and chronic wounds. You will play a key role in improving patient outcomes through evidence-based practice, clinical consultation, education and support of staff managing complex wounds across community settings. The successful applicant will demonstrate highly developed communication and interpersonal skills, enabling effective collaboration with a broad range of clinicians and specialists both within and outside the public health system. You will contribute to the formulation and coordination of treatment and management plans aligned with current best practice, while ensuring patient involvement in decision-making, often within complex and challenging socio-economic circumstances.
Northern Sydney Home Nursing Service (NSHNS) is a generalist Community Nursing service, delivering care to clients in their homes and clinic environments. The service provides a broad range of clinical care including comprehensive assessments, care coordination and referrals to appropriate services and specialists based on patient needs. Clinical staff deliver medication administration (oral, parenteral and topical), wound care ranging from simple to highly complex, palliative and end-of-life care, syringe driver management, chronic disease monitoring (including respiratory and heart failure), continence assessments, indwelling and suprapubic catheter care, central venous access device (CVAD) management and more.
The NP/TNP will work closely with and support NSHNS Community Nursing staff, enhancing capability in complex wound management while contributing to holistic patient care. Patients managed within this role are often also under the care of generalist Registered Nurses and may present with multiple, co-existing health needs beyond wound care. You will provide specialist input while working collaboratively with the broader team to ensure coordinated, comprehensive and person-centred care across the continuum.

Candidates will need to meet the following criteria:
- Current registration as a Registered Nurse with endorsement to practice as a Nurse Practitioner with the Australian Health Practitioners Regulation Authority (AHPRA) or enrolled in an approved Nurse Practitioner Masters Degree working towards endorsement with a minimum of 5 years post registration experience with at least 3 years in the specialty.
- Demonstrated effective written and verbal communication skills, with the ability to work both independently and collaboratively within multidisciplinary teams.
- Demonstrated computer literacy and ability to utilise clinical systems effectively, with a current NSW Driver Licence and capacity to work across community-based settings.
