Where you'll be working:

The Nurse Practitioner – Residential Aged Care In-Reach (RAC-I) will be based at Newcastle Community Health Centre, delivering specialised care across Residential Aged Care Facilities (RACFs) in Newcastle, Lake Macquarie, Port Stephens, and Taree—with future expansion anticipated. The service is embedded within a collaborative multidisciplinary team comprising Geriatricians, Medical Officers, nursing and allied health professionals, all working together to enhance quality of life for aged care residents.

The RAC-I service aims to deliver timely, patient-centred, urgent care directly in residential settings through a hybrid model of virtual and in-person care. By reducing unnecessary hospital transfers and supporting continuity of care, the model supports residents to remain in familiar surroundings while ensuring they receive expert assessment and treatment.



About the role:

This is an advanced clinical role delivering high-level nursing care in the specialty area of acute aged care. The Nurse Practitioner will assess, diagnose, treat and monitor RACF residents, helping prevent unnecessary hospital presentations by providing safe and effective in-reach care. This service works closely with the Newcastle Hospital in the Home (HITH) team to complement and extend acute care access for aged care residents.

You’ll be supported by the Director of Nursing – Integrated Care Service Hunter & Lower Mid North Coast, with additional clinical guidance from the Director of Geriatric Medicine and colleagues. A local governance committee and the HNELHD Nurse Practitioner Network will also assist in developing your scope of practice and ensuring a strong support system for clinical and professional development.

You thrive working both autonomously and within a multidisciplinary team, and your communication and leadership abilities allow you to build trust and achieve timely, person-centred outcomes.

You bring strong critical thinking and clinical decision-making capabilities, and your drive for ongoing learning reflects your dedication to delivering the highest quality care.



Requirements:

  • Authorisation as a Nurse Practitioner by the Nursing and Midwifery Board of Australia
  • Significant clinical experience in aged care, acute care, or similar environments
  • Demonstrated ability to work independently and collaboratively in a multidisciplinary team
  • Current unrestricted NSW driver’s licence and willingness to travel within the region
