Working within a multidisciplinary team, this position will provide recovery oriented clinical rehabilitation interventions with people aged 16-65 and living with mental health issues in the community. This position provides: assessment and rehabilitation care planning, skills development and education, VETE, health promotion, specialist therapies and referral and linkage to relevant agencies as appropriate.
