The Mental Health Disability Navigator is a specialist subject matter expert providing high-level operational advice, leadership, and coordination to frontline mental health staff supporting consumers who require disability services to live safely in the community.
The role supports staff to navigate the NDIS and its interface with NSW Health, translating complex policy into practical guidance to improve consumer outcomes and service efficiency.
The position sits within the Pathways to Community Living Initiative (PCLI) team and works across inpatient and community settings. It works in partnership with the ISLHD Disability Navigator (District NDIS Lead) to maintain consistent practice and effective communication and escalation pathways with the NDIA.
The role contributes to workforce capability building, reducing delayed discharge and non-clinical length of stay, lowering inpatient demand, and preventing avoidable admissions through early identification and coordination of disability supports.

Shellharbour Hospital is a busy acute care facility which provides emergency services, elective surgical services, general medicine, antenatal and primary family health care, satellite renal dialysis, and adolescent and adult mental health services on site. It's an accredited healthcare facility with the Australian Council on Health Care Standards (ACHS) and is recognised as providing a high standard of care to the community.
Located on the beautiful South Coast of NSW, Shellharbour is known for its spectacular coastal scenery and is only a little over a 1.5 hour drive from Sydney.
