What you'll be doing
Role as care coordinator for OPMHS includes role of intake person for new referrals. Using skills in conducting comprehensive mental health assessments including risk assessments associated with suicide, self harm, reputational risk, and risks associated with expoitation and abuse.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria.
- Current Australian Health Practitioner Regulation Agency (APRHA) registration as a Registered Nurse, Psychologist or Occupational Therapist or eligibility for membership to the AASW. Recognised by the Diversional Therapy Association of Australian National Council, or other qualification deemed equivalent by the employer.
- Demonstrated skills in conducting comprehensive older persons mental health assessment and management, including assessment, risk management and care coordination.
Demonstrated capacity to comprehensively assess and manage risk associated with suicide, deliberate self-harm , reputational risk and risk associated with violence, exploitation and abuse.
Demonstrated understanding of the Recovery Model of care within Mental Health and the ability to implement and evaluate these care processes within the context of clinical care planning.
