Selection Criteria
- Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia.
- Minimum five years full time equivalent post graduate qualification experience and a post graduate qualification in a relevant field of practice, or such other qualifications or experience deemed appropriate.
- Demonstrated experience working with and extensive knowledge of the broader health and social issues that people with chronic and complex conditions require.
- Demonstrated ability in communicating effectively with multiple care providers including general practitioners, other primary care providers, and members of multidisciplinary teams as required.
- Demonstrated effective computer skills in using electronic medical records and the Microsoft suite of products (Outlook, Word, Excel, PowerPoint).
- Demonstrated clinical expertise and the ability to apply clinical expertise in person-centred care planning.
- Demonstrated ability to work independently and within a multidisciplinary team.
- Current unrestricted Class C NSW Driver's Licence.
