What You'll Be Doing:
This CNC works in the dynamic ED to Community team of the Integrated Care unit, working predominately in the community. The ED to Community team works to improve individual patient outcomes and experiences by coordinating healthcare services, especially for those with complex needs who frequently use the Emergency Department. This involves co-ordinating care and developing partnerships across hospitals, GPs, community health services, and other relevant services to provide a more effective care pathway.

The Integrated Care CNC promotes an integrated person-centred care model which advocates for effective and efficient care focusing on the unique physical, mental and social needs of the individual and fosters a collaborative partnership between patients/carers, service providers and their General Practitioner/primary care to optimise health outcomes and develop appropriate care plans. The Integrated Care CNC plays a major clinical role through the provision of expert clinical assessment, referral and coordination of health and community services for people with severe chronic disease and complex care requirements living in the community who are at risk of deteriorating health that may result in declining quality of life and an acute hospital admission.

The Integrated Care CNC provides expert clinical advice to patients, carers and other health care professionals within SESLHD, community organisations, primary care/General Practice, Non-Government Organisations (NGOs) and other government departments to support integration of care.

The Integrated Care CNC utilises a flexible and adaptive approach to patient care within a changing health system and supports/coaches patients to coordinate and navigate across the health care system and to connect with other community and hospital-based coordination services as required in drug and alcohol, mental health and primary care.

Selection Criteria:

 

  1. Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia.
  2. Minimum five years full time equivalent post graduate qualification experience and a post graduate qualification in a relevant field of practice, or such other qualifications or experience deemed appropriate.
  3. Demonstrated experience working with and extensive knowledge of the broader health and social issues that people with chronic and complex conditions require.
  4. Demonstrated ability in communicating effectively with multiple care providers including general practitioners, other primary care providers, and members of multidisciplinary teams as required.
  5. Demonstrated effective computer skills in using electronic medical records and the Microsoft suite of products (Outlook, Word, Excel, PowerPoint).
  6. Demonstrated clinical expertise and the ability to apply clinical expertise in person-centred care planning.
  7. Demonstrated ability to work independently and within a multidisciplinary team.
  8. Current unrestricted Class C NSW Driver's Licence.
