Where you'll be working

You will primarily be based in HITH Clinic at Royal North Shore Hospital (RNSH) Campus. Occasionally, you may be required to travel within NSLHD depending on service need.

What you'll be doing

Leading the HITH clinic at RNSH, ensuring efficient patient flow, customer service and quality care. Providing acute-level care to patients in the community to prevent hospital admissions, reduce ED presentations and facilitate early discharges from hospitals. Working within a multi-disciplinary team to provide the highest standard of care. Collaborating with medical teams and guiding the practice of RNs in HITH clinic.

Collaborating with our key referring stakeholders during the onboarding and subsequent care of patients on the HITH service. Supporting the HITH Intake and HITH Liaison role at times.

Candidates will need to meet the following criteria:

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Current registration with AHPRA as a Registered Nurse Division 1, with post registration qualification relevant to specialty and at least 3 years’ experience working in the clinical area of the post graduate qualification or such other qualifications or relevant equivalent experience.
  3. Demonstrated ability to communicate effectively with a diverse range of health care providers including those in government, non-government, and residential aged care and community organisations.
  4. Evidence of advanced level of knowledge and skill related to Hospital in the Home and demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.
  5. Demonstrated excellence in written and verbal communication skills and computer literacy in Microsoft Applications and evidence of personal time management capabilities and effective negotiation skills.
  6. Demonstrated knowledge in initiating and implementing changes to improve clinical service delivery and outcomes, models of care, evidence based practice and practice development.
  7. Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community based services and a comprehensive knowledge of primary health and community based services.
  8. Current NSW driver’s licence
