About the role
We are seeking a committed Health Clinician to join our multidisciplinary Aged Care Services team and play a central role in coordinating high‑quality, person‑centred care for older adults.

The role supports the Single Aged Care Assessment Service by providing comprehensive assessments and guiding clients toward the most appropriate supports. Work closely with consumers, carers, and a wide network of providers to deliver integrated clinical services that promote independence, restorative care, and reablement.

This role offers the opportunity to advocate for older people, support their wellbeing, and help them remain safe and independent in their homes. Be part of a supportive leadership team within a service that is evolving in response to a growing ageing population and ongoing Commonwealth and State reforms.
