About the Opportunity 
There are two exciting opportunities existing for an experienced health clinician to join an established Disability Resource Team in Bankstown Hospital until 16/01/2025 and Bolger Street, Campbelltown until 03/04/2025 as a Disability Resource Coach to improve the care experiences for people with disability, their families and informal supports. 

This role supports the care experience of people with disabilities and their carers across South Western Sydney Local Health District facilities and services.  The Disability Resource Coach works collaboratively with facility multidisciplinary teams and external stakeholders to address disability related needs, establishing reasonable adjustments and supporting discharge planning.

 

What You'll be Doing
Improve the care experience for people with disability, their families and informal supports.  
Facilitate continuity, coordination and transfer of care between admitted and community settings. 
Work collaboratively with SWSLHD and external partners to ensure the care needs of people with a disability are met such that fewer acute health responses are needed. Provide guidance, leadership and advice to facilities to support the needs of people with disabilities who present to hospital. 
Implement proactive care navigation and hospital avoidance strategies for people with disabilities with unmet/unstable care needs. Build and strengthen networks with SWSLHD and external services that support the needs of people with disability. Undertake duties and reporting requirements associated with the position.

How to Apply

  1. Relevant qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA)qualified Social Worker eligible for membership with the Australia Association of Social Workers (AASW) or qualified Speech Pathologist eligible for membership with Speech Pathology Australia. CNC Grade 1:  At least 5 years full time equivalent post registration experience and in addition who has approved post registration nursing/midwifery qualifications relevant to the field in which the employee is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation. 
  2. Demonstrate your in-depth understanding of the health and disability sectors as a health clinician supporting clients within the National Disability Insurance Scheme (NDIS). Please provide examples of how you have coordinated care and navigated systems for individuals with complex needs—particularly those exhibiting behaviours of concern—across multiple services and agencies.
  3. Describe your understanding of the principles of access and reasonable adjustment for people with physical, cognitive, communication, psychosocial, and sensory disabilities within healthcare settings. Please provide specific examples of how you have identified barriers and implemented practical adjustments to ensure equitable participation and inclusion of patients or consumers throughout their healthcare journey.
  4. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
