Community Mental Health Clinician, Care Coordination Team
We are looking for a motivated mental health clinician to work Part Time in our Community Mental Health Rehabilitation Team.
Duties will include recovery oriented practice based assessments, collaborative goal/ treatment planning/ provision, and care coordination for allocated consumers and assisting with the development, implementation & evaluation of the DBT program.
Attendance at supervision & CPD activities are strongly encouraged/supported.

What you'll be doing
To provide nursing and health services as a member of a multi-disciplinary community mental health team which offers triage, assessment, treatment, ongoing case management and discharge planning services to individuals in Northern Sydney Local Health Districts.
Candidates will need to meet the following criteria:
- Current AHPRA registration for relevant discipline - Psychology, Occupational Therapist, Registered Nurse, Eligibility for membership with AASW for Social Workers.
- Demonstrated experience in comprehensive mental health assessment and biopsychosical interventions including DBT.
Current NSW Driver's licence.
