About the role
The Discharge planner provides advanced, specialist nursing care to support safe, timely, and person centred discharge planning for patients with complex needs across inpatient areas. This role contributes to improved patient flow, reduced discharge delays, and continuity of care from admission through to discharge.
The position requires strong clinical expertise, effective communication, and leadership skills, with the ability to work collaboratively with multidisciplinary teams, patients, and families. The role leads and participates in daily MDT reviews and patient flow activities, identifies and addresses discharge barriers, and provides specialist advice and support to staff.
This role works Monday to Friday during business hours. The role offers opportunities to influence patient outcomes, support service improvement, and contribute to high quality, coordinated care.
