The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Through proactive care navigation, seamless coordination, and tailored health coaching, the CNS2 works across diverse programs to ensure every client receives timely, integrated support. Strong professional relationships with General Practitioners, health care providers, and stakeholders are key, with care delivered both in-person and via virtual platforms.

What you will bring to the role
- Current registration as a Registered Nurse Division 1 AHPRA with relevant post-registration qualifications and at least 3 years’ experience working with clients who have a chronic condition and/or complex care needs or such other qualifications or equivalent experience deemed appropriate.
- Demonstrates a high level of confidence and clinical proficiency in the provision of care navigation, care coordination and health coaching
- Demonstrated ability to effectively communicate and engage with clients, families/carers and the healthcare team to improve clinical service delivery, client outcomes and provide safe quality care that is culturally appropriate
- Demonstrated ability to work as a member of a multidisciplinary team and to communicate effectively with a broad range of health professionals/ services and social services to facilitate the coordination of comprehensive and collaborative care
- Demonstrated problem solving and decision-making abilities and high level communication, liaison, negotiation, time management and report writing skills
- Demonstrated ability to provide leadership in the provision and development of clinical practice, offer expert advice in the management of chronic and/or complex clients and has a demonstrated commitment to own professional development
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and commitment to undertake training in electronic health record systems
- Current valid Driver's Licence and willingness to use for work purposes. This may include driving long distances across the LHD
