An exciting opportunity has arisen to work in a multidisciplinary rapid response geriatric outreach team based on the Northern Beaches.

Remuneration: $57.11 - $58.99 per hour

What you'll be doing
Acute Assessment and treatment of older people, with a view to prevent hospital presentations/ED avoidance.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current Registered Nurse, registered to practice with AHPRA. Hold relevant post graduate qualifications with at least 3 years’ recent experience in the speciality of Aged Care and /or Community Nursing or such other qualifications or relevant equivalent experience.
- Demonstrated well developed written, verbal and interpersonal communication skills, including computer skills, with demonstrated experience in or willingness to learn and use electronic clinical/medical/patient information and reporting systems.
- Demonstrated ability to work effectively and problem solve both independently and as a member of the multidisciplinary team.
