About the Opportunity

• Provide holistic assessment, counselling, advocacy and case management support to clients and their carers.
• Deliver evidence-informed social work services across community and outpatient settings.
• Collaborate with a multidisciplinary team including allied health professionals, medical staff and community agencies.
• Support patients experiencing the emotional and social impacts of illness and complex health conditions, including those with a life limiting condition.
• Contribute to service development, quality improvement, education and student supervision activities.
• Work across multiple Central Coast locations to ensure timely and responsive service delivery.

About you

• Degree qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
• Strong interpersonal skills with the ability to engage effectively with clients, carers and healthcare professionals.
• Ability to manage a diverse caseload while prioritising competing demands in a dynamic environment.
• Demonstrated commitment to person-centred, evidence-based practice and professional standards.
• Experience working collaboratively within multidisciplinary teams.
• Resilient, adaptable and able to support clients experiencing distress, anxiety and complex psychosocial needs.
