About the role:
The Carer Network Coordinator – Integrated Care will function as a member of the MLHD Integrated Care Team. As the Carer Network Coordinator, you will develop and lead the coordination of Community Carer Services, ensuring that the planning, organisation and scheduling of carer services supports MLHD organisational goals.
You will provide high level advocacy around carers with a particular focus on empowering carers to provide care for their loved ones within the community and building of a carer support network.
The Carer Network Coordinator will facilitate development of partnerships with carers, consumers, clinicians, managers, government and non-government agencies as well as community organisations and support groups.
Employment Type: Temporary Part Time (until 28 June 2026)
Position Classification: Dependent upon qualifications and experience (refer to PD)
Remuneration: $116,639.98 - $120,680.00 per annum, pro rata (+ super +17.5% leave loading where applicable)
Hours Per Week: up to 12

Our ideal candidate will demonstrate:
Relevant tertiary qualifications in health (relevant post graduate Nursing qualification to work as a Clinical Nurse Specialist Grade 2 OR demonstrated capacity to work professionally as an Occupational Therapist Level 3, Physiotherapist Level 3, Dietitian Level 3, Speech Pathologist Level 3 or Social Worker Level 3.)
AHPRA Registration of eligibility for membership to relevant peak body
Strong interpersonal; written and verbal communication; and consultation skills
