Remuneration: $147,653.00 – $175,917.00 per annum (+ super + 17.5% leave loading where applicable)

About the Opportunity
We are seeking an experienced Program Manager – Aged Care & Out of Hospital Services to provide high-level strategic leadership across a diverse and essential service portfolio. This is a critical leadership role responsible for the planning, implementation, monitoring and evaluation of aged care and out-of-hospital programs across the district, ensuring services remain safe, sustainable and responsive to community needs.
Program Manager - Aged Care & Out of Hospital Services, you will:
- Provide strategic leadership and coordination across Aged Care Assessment Services, Transitional Aged Care Program (TACP), Commonwealth Home Support Program (CHSP), Out of Hospital Care Programs and the Aged Care Outreach Service (ACOS).
- Plan, implement and review service models to ensure alignment with Commonwealth, State and local policy and funding requirements.
- Manage contracts and funding arrangements with external service providers, including performance monitoring and service evaluations.
- Oversee governance, accreditation, policy development and compliance, including My Aged Care portal administration and CHSP accreditation processes.

About You
Our ideal candidate will demonstrate: - Extensive senior leadership experience within aged care, community health or complex health service environments.
- Strong knowledge of Commonwealth and State aged care policy, funding models and accreditation requirements.
- A leadership style grounded in empathy, accountability and a commitment to high-quality, patient-centred care.
