The Role

The Aboriginal Supportive and Community Care Coordinator role will: 

  • Support Aboriginal clients and their families using SESLHD services with Advanced Care planning and access to palliative and end of life service.    
  •  Engage local Aboriginal communities in the development, delivery and evaluation of culturally appropriate initiatives and strategies that provide positive Palliative and end of life care outcomes.
  • Collaborate with SESLHD Palliative Care services, primary care and other health and social care services to develop culturally appropriate, integrated and comprehensive care plans.
  • Build mainstream services’ understanding of local Aboriginal communities and their needs around Cultural practices.

Selection Criteria

  1. Be of Aboriginal or Torres Strait Islander descent (An applicant's race is a genuine occupational qualification and is authorised under section 14(d) of the Anti-Discrimination Act 1977).
  2. Holds a minimum Certificate III qualification in Aboriginal Primary Health Care or relevant qualifications/experience in an appropriate health related field.
  3. Demonstrated understanding of issues affecting Aboriginal people in relation to grief and loss and Aboriginal social and emotional well being.
  4. Experience in community engagement and/or development with Aboriginal people .
  5. Knowledge of advance care planning and end of life services.
  6. Demonstrated ability to work independently and as part of multidisciplinary team to develop culturally safe environment for Aboriginal clients and their families.
  7. Demonstrated excellent written and verbal communication skills with the ability to develop culturally appropriate resources and document in the client’s electronic medical record.
  8. Current unrestricted driver’s licence (with a willingness to travel in accordance with the demands of the position).
