Where you'll be working

Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
 
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
 
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

The Role
 

The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it. 
  
Provide a high-level speciality support for the management of Aboriginal population health programs and projects to support the efficient and effective operation of the Equity and Prevention Service and Public Health Unit to achieve business and service objectives, including health outcome action plans for diabetes prevalence in adults, and immunisation in adolescents and adults.

The Aboriginal Population Health Programs Coordinator will lead projects to address needs of the Aboriginal population as relevant to Population and Community Health (PaCH) services. 
  
The role will support collaboration and coordination of Aboriginal population health initiatives across PaCH services. The role will work closely and collaboratively with the SESLHD Aboriginal Health Directorate.

  1. Being of Aboriginal and/or Torres Strait Islander descent (An applicant’s race is a genuine occupational qualification and is authorised under section 14(d) of the Anti-Discrimination Act 1977) .
  2. Relevant tertiary qualifications in Aboriginal Health, health promotion, public health; or relevant equivalent work experience or a combination of study and work experience.
  3. Demonstrated experience in Aboriginal Health; as well as planning, implementing and evaluating population health programs and initiatives with Aboriginal communities.
  4. Demonstrated high level verbal and written communication skills, interpersonal, negotiation and influencing skills with a strong customer service approach
  5. Demonstrated high level analytical and problem-solving skills that lead to the development of innovative solutions to the delivery of population health initiatives.
  6. Demonstrated ability to develop and maintain effective working relationships with senior management and other key stakeholders
  7. Demonstrated ability to assist in the development of policy and processes in accordance with approved guidelines.
  8. Current drivers licence with a willingness to travel in accordance with the demands of the position
