Where you'll be working

SESLHD Mental Health Service, located at Kirk Place, Kogarah. Travel to other Mental Health Service sites and across the SESLHD will be required.

The Aboriginal Mental Health Care Navigator will support the systems, services and processes between Mental Health and non-government organisations, primary care, social support, justice and drug and alcohol services to ensure collaboration and, to reduce service duplication and ensure appropriate support at transitional points in care for Aboriginal people. 
  
The Care Navigator will co-ordinate a range of preventative, health and social care wrap around responses for Aboriginal consumers. The Care Navigator will ensure services build on the strengths, resources and knowledge of Aboriginal consumers, be responsible for supporting the broader mental health service system (including CMOs) to increase their capacity to work with Aboriginal people, families and communities in a culturally safe manner.

Selection Criteria: 

  1. To be of Aboriginal and/or Torres Strait Islander descent. Aboriginality is a genuine occupational qualification and is authorised by section 14(d) of the Anti-Discrimination Act 1977.
  2. Relevant tertiary qualifications in health or management related field or relevant equivalent work experience, or a combination of study and work experience as deemed appropriate by Manager of the position.
  3. Demonstrated knowledge and understanding of complex issues that impact on the health and wellbeing of Aboriginal people.
  4. Demonstrated ability to work independently or as part of a team in performing a range of skills within allocated timeframes.
  5. Demonstrated effective interpersonal and communication skills to collaborate with key stakeholders.
  6. Demonstrated excellent written and verbal communication skills with the ability to negotiate and influence to improve health outcomes.
  7. Demonstrated ability to be flexible and adapt work practices to meet current and emerging consumer and community needs.
  8. Current drivers licence (with a willingness to travel in accordance with the demands of the position).
