Join our team as an Aboriginal Health Worker and make a meaningful difference in the lives of Aboriginal patients, families and communities. In this important role, you will contribute to improving health outcomes for Aboriginal people by providing culturally safe, patient‑centred care coordination to individuals living with chronic or complex health needs. Working closely with clinicians and other health professionals, you will support patients in navigating the healthcare system, ensuring their clinical, social and cultural needs are recognised and respected. This role offers the opportunity to strengthen cultural safety within our services, empower individuals through advocacy and education, and play a vital part in supporting holistic care across the patient journey.
Are You the Right Fit?
You are a compassionate and dedicated Aboriginal Health Worker who is committed to delivering culturally safe, patient‑centred care to Aboriginal patients and their families. You work collaboratively with clinicians to provide comprehensive assessment, care coordination and education that reflects the clinical, psychosocial and cultural needs of each individual. You are passionate about supporting patients to access timely and appropriate services, advocating for their needs and helping them navigate complex health pathways. Your ability to build trust, empower communities and uphold cultural integrity will be central to your success in this role.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
1. In this role being an Aboriginal and/or Torres Strait Islander person is a genuine occupational requirement. Tell us about your mob and connection to culture?
2. Certificate III in Aboriginal and Torres Strait Islander Primary Health Care or relevant qualification or willingness to complete and to undertake further training as identified.
3. Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal communities.
4. Demonstrated experience delivering culturally safe health services to Aboriginal communities.
5. Demonstrated written and verbal communication skills and computer literacy.
6. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
