About the Role

This is a deeply meaningful role that supports Aboriginal patients and their families through palliative care, end of life care, and life-limiting illness. Working across both hospital and community settings, you will provide holistic and culturally appropriate care, ensuring cultural, family, and community needs are recognised, respected, and embedded in care planning and decision-making.

You will work alongside a multidisciplinary palliative care team, Aboriginal health staff, and community organisations to improve access to services, provide education and advocacy, and support patients and families during what can be an incredibly challenging time. The impact of this role extends beyond the individual and into families and the broader community.

 

What You’ll Be Doing

In this role, you will provide culturally centred primary health care within your scope of practice and support Aboriginal patients and families navigating palliative and end of life care. You will help create clear and supportive pathways to specialist services, advocate for culturally safe care, and support shared decision-making. You will also maintain a visible presence within the local Aboriginal community, offering education, guidance, and support, while working closely with the multidisciplinary team to ensure care is coordinated, respectful, and responsive.

This role requires compassion, resilience, and the ability to care for yourself while caring for others. You will feel comfortable working in both hospital and community environments and be able to adapt your approach to meet the needs of patients, families, and communities.
 

Support

You will be supported by the Social Work Manager, Allied Health Professionals, the Palliative Care NUM, and the wider Social Work team. Orientation and training will be tailored to your experience, particularly for those who are new to working within NSW Health. Ongoing education and professional development are strongly encouraged and supported, and are a valued part of our team culture.

Requirements:

  • Hold a Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).

  • Eligible to drive in NSW and the ability to travel as required.
