What you'll be doing
You’ll be a trusted point of contact for Aboriginal people diagnosed with cancer, supporting them and their families every step of the way while advocating for culturally responsive care.
In this role its important that you will be able to:
Walk alongside patients and families, supporting them through diagnosis, treatment and follow-up
Help patients and families navigate the health system and connect with the right services
Identify and reduce barriers like transport, accommodation and communication
Ensure cultural and psychosocial needs are recognised and supported, including referrals where needed
Work with teams to improve care pathways and strengthen culturally safe practices

Essential Criteria:
Evidence of Diversity claim if applicable (state which category): Confirmation of Aboriginality.
Selection Criteria:
Hold qualifications in Aboriginal primary health care or other health related discipline, or equivalent experience.
Demonstrated active involvement and lived experience within Aboriginal communities, accompanied by a deep understanding and strong commitment to addressing health issues affecting Aboriginal peoples.
Demonstrated experience providing care coordination or case management support for clients with complex needs.
Demonstrated ability to work as part of a multidisciplinary team and ensure cultural needs of Aboriginal patients and their families are identified and addressed.
Ability to establish and maintain strong stakeholder relationships, with both mainstream and Aboriginal services to support care coordination.
Highly developed written and verbal communication skills, including proficiency in word processing and email, with the ability to prepare clear, concise reports and correspondence tailored to diverse audiences.
Valid NSW Drivers License and be willing to use.
