Remuneration: $113,243.15 - $116,963.48 per annum

Where you'll be working 
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Cumberland and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
This role plays a vital part in improving access to health care for marginalised young people, with a focus on those who frequently present to Emergency Departments.

  • Support marginalised young people to better navigate the health care system
  • Focus on enhancing access to primary and community health services
  • Reduce reliance on Emergency Departments by improving care pathways
  • Build strong, trust-based relationships with young people facing barriers to care
  • Identify and address systemic gaps in access to ongoing, coordinated health support
  • Collaborate with stakeholders to develop a framework that improves access for this cohort
  • Contribute to long-term strategies that improve equity and health outcomes for vulnerable youth

Essential Requirements

  • Registration: Current APHRA Registration – registered Nurse/Midwife
  • Qualification: Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.  




Selection Criteria

  1. Demonstrated understanding of and experience working with young people including young people from marginalised and at risk groups.
  2. Demonstrated experience working collaboratively with other service providers including General Practitioners, Community Services, Non-Government Organisations and the ability to work within a multidisciplinary team.
  3. Demonstrated understanding of NSW Health initiatives for Youth Health, NSW Government Youth Initiatives and ability to implement the principles of Primary Health Care.
  4. Demonstrated high level communication, liaison, negotiation, time management, computer and presentation skills.
  5. Knowledge of and/or experience in Acute service settings.
  6. Research & evaluation experience, including data collection and data entry.
  7. Post- registration qualifications and at least 3 years’ experience working in the clinical area of the specified post graduate qualifications.
