About the Role
The opportunity exists for a suitably experienced pharmacist to join the dynamic Pharmacy service within the Sydney Local Health District (SLHD) based at Royal Prince Alfred Hospital (RPAH). This permanent full time position reports to the SLHD Director of Pharmacy and RPA Director of Pharmacy, and also provides services to Balmain Hospital. Significant aspects of the position are detailed in the position description and include maintaining optimal venous thromboembolism prophylaxis, anticoagulant management and facilitating other quality use of medication initiatives and reviews.
RPAH provides a comprehensive range of clinical services to residents of the SLHD and has specialised areas of practice in liver, renal and haemopoietic stem cell transplant, Melanoma therapy, Haematology, Cardiology, Mental Health and advanced surgical procedures. The pharmacy department provides clinical trials, outpatients and production unit services and is a leader in the deployment of novel and innovative models of clinical care in addition to VTE Stewardship.

Ideal Candidate
- Current registered as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA) and able to practice independently.
- Understanding of the priorities, and requirements of a VTE/Anticoagulation Stewardship program and quality improvement projects, such as drug use evaluations, with consideration of barriers to successful implementation.
- Experience in developing, reviewing and implementing medication guidelines, policies or reports and ability to use clinical evidence, applicable data and judgement to communicate recommendations to influence practice change or treatment decisions made by clinicians to improve patient outcomes.
