What you'll be doing

  • The Voluntary Assisted Dying Care Navigator assists patients and staff to navigate Voluntary Assisted Dying (VAD) ensuring equitable, well-coordinated and person centred service delivery in-line with NSW Voluntary Assisted Dying Legislation across the Far West Local Health District (FWLHD).
  • Work closely with the State-Wide Voluntary Assisted Dying Support Services (3 State-Wide Services; Care Navigation Service , Access Team and Pharmacy) who will be part of VAD service delivery in the Far West Local Health District. 
  • This position is district wide and provides outreach services to enable patients to receive care within their local community or place or place of residence. 
  • Responsible for ongoing education and support FWLHD clinicians and primary care services while also fostering stakeholder engagement to enable equitable access for people wishing to access VAD services.

How to apply:
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible.

  • Registered Nurses with a minimum of 3 years' experience working in a relevant clinical area and hold a related post graduate certificate.
  • Experience participating in end-of-life conversations with patients, families, and clinical colleagues, and understanding of the sensitivities and dynamics entailed in end-of-life decisions and of the end-of-life care landscape in NSW.
  • Empathic and supportive approach to client enquiries and follow up.
  • Reasonable computer skills, such capability in accessing databases and data entry.
  • Demonstrated networking, negotiation and problem solving skills with ability to work collaboratively with stakeholders to achieve a common goal.
  • Knowledge and understanding of the New South Wales health system and NSW Ministry of Health policies and procedures with a sound appreciation of the NSW Voluntary Assisted Dying Act 2022.
  • A current unrestricted drivers' license with ability and willingness to travel as the role requires. This may involve driving long distances and overnight stays.
