The Registered Nurse works within the Mission and Mission in Action of HammondCare to ensure safe, effective, quality patient care.


About the role

The Transitional Nurse Practitioner (TNP) works within the Mission and Mission in Action of HammondCare to provide expert clinical support of the activities of the Community Palliative Care Team North. The TNP is an expert in palliative care with aged care assessment skills and knowledge. Additionally the TNP is to provide support of the educational and professional development of nursing groups, palliative care staff, patients, carers and their families.

A core function of the role is to be a conduit for residents of aged care facilities or other hospitals transitioning to end of life care and establishing links with appropriate clinical services within HammondCare. The TNP is able to extend nursing practice by facilitating timely patient review and manage patients that present with palliative/end of life care needs.


About you

  • Registered Nurse willing to enrol in a Tertiary Master’s program for Nurse Practitioner. Minimum of 5000 hours of advanced nursing/midwifery practice during the last six years appropriate to the specialty area (or working towards within the next 3 years).
  • Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.
  • Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
  • Demonstrated ability to work autonomously and have effective communication and negotiation skills.
  • Demonstrated ability to work independently within the wider multidisciplinary team.
  • Demonstrated effective communication and interpersonal skills (written and verbal)
  • Demonstrated organisation, networking and time management skills
  • Demonstrated ability to work with sensitivity, empathy, patience and flexibility.
  • A current drivers license and safe driving record. 
  • Willingness to travel between HammondCare’s sites and services.
