What you'll be doing
As the Transition & Physical Health Coordinator, you will provide expert clinical leadership and care coordination to support consumers experiencing mental health challenges across their recovery journey. Working within a multidisciplinary team, you will facilitate safe and effective transitions between services, promote holistic care that addresses both mental and physical health needs, and champion best-practice approaches to service delivery.

You will utilise your advanced clinical expertise to assess, plan, coordinate and evaluate care, ensuring consumers receive person-centred, recovery-focused support. This role offers the opportunity to influence clinical practice, contribute to service improvement initiatives, and work collaboratively with consumers, carers, community partners and healthcare professionals to achieve positive health outcomes.


About You

You are an experienced and passionate mental health clinician who is committed to delivering person-centred, recovery-focused care. With strong clinical leadership skills and the ability to work collaboratively across multidisciplinary teams, you bring advanced mental health expertise, sound clinical judgement, and a thorough understanding of contemporary mental health legislation and best practice. You are confident using clinical information systems, hold a current unrestricted driver's licence, and are committed to improving outcomes for consumers through holistic, coordinated care.

IMHAOD Statement of Intention 
The intention of the MNCLHD Integrated Mental Health and Alcohol and Other Drugs Services (IMHAOD) is to provide recovery-oriented, trauma informed services that are guided by evidence-based practices and collaboration. 
  
The Transition and Physical Health Coordinator is primarily responsible for two primary functions.  

  1. Within the wider multidisciplinary team to coordinate, negotiate, and manage discharge planning needs of consumers to facilitate a smooth transition from hospital to the Community Mental Health Service and coordinate 7-day post discharge follow up clinics
  2. Coordinate physical and metabolic care pathways ensuring the physical health of consumers is monitored and interventions are put in place.
