The Role

As a CNS2 2 in SESLHD: 

  • You will provide individualised, case management of complex patient care.
  • Utilise clinical skills and critical thinking in delivering high quality patient care. Display autonomy of decision making and professional knowledge and judgement
  • You will lead the development of the nursing specialty clinical practice and service delivery in the ward/unit/service.
  • You will be an integral part of the interdisciplinary team, and a team that strives to support eachother.
  • Ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate.
  • Share your clinical expertise and further develop your practice.


Some of the key functions of your role will be: 

  • Participate in rapid response/ code blue/ clinical reviews
  • Provide 24/7 expert clinical advice and support
  • Collaborate with the ICU Medical Outreach team to provide coordinated approach to outreach services
  • Coordination and monitoring of the acute care hospitalisation phase of the ICU patient in conjunction with the ICU Outreach CNC
  • Follow up discharged ICU patients after transfer to the ward to assess clinical condition
  • Identify & provide assistance and support to ward staff in determining care needs as well as managing any risks in deterioration,
  • Review after hours admissions to ward from ED of patients deemed unsuitable for ICU to assess clinical condition and provide assistance and support to ward staff in determining care plan

Selection Criteria

  1. Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife
  2. Relevant post-registration qualifications and at least 3 years’ critical care experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.
  3. Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans.
  4. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
  5. Demonstrated skills in the development and facilitation of patient and carer education; and ability to mentor and coach nursing staff to enhance professional development.
  6. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
  7. Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
  8. Demonstrated competence in the use of health-related information technology.
