The Role

The Acute Transition Team (ATT) has been established to support individualised patient discharge planning, sustainable safe care and good communication for patients transitioning from one level of care to another, including discharge to home or another care provider in the community. 

The role of the ATT comprises a CNC2, Occupational Therapist and Social Work and is to ensure processes and clear timeframes are in place for discharge planning as patients’ transition through each phase of their care. The transition team will by provide support and expertise to clinical staff in managing the patient journey and mitigating avoidable discharge delays. 

The role of the team is to provide support and assist with the management of: 

  • Complex patients in SGH in consultation with Divisional and patient flow staff
  • Patients on an RACF pathway
  • NCAT and Guardianship matters
  • Patients on an NDIS pathway
  • Vulnerable sub specialty patients


CNC2 roles and responsibilities: 

  • Exercises extended autonomy decision making, professional knowledge and experience in providing complex care requiring advanced clinical skills in relation to complex care coordination
  • Develop, facilitate, implement and evaluate care management plan for patients with complex health needs
  • Identification of vulnerable patients early in episode of care at risk of complex discharge, identified by proactive and autonomous investigation or by referral from Multi-Disciplinary teams.
  • Maintains high profile, visibility and leadership to guide Clinical teams to assist in problem solving, and managing complex discharge planning  to assist with a positive patient experience and patient flow
  • Supporting coordination  of complex care and  by assisting  Transition team OT & SW on NDIS, RACF and guardianship planning
  • Tracking  RACF delays and attending  weekly RACF meetings, provision of advanced clinical advice
  • Attending priority MDT meetings with provision of advanced clinical advice
  • Using Clinical consultancy knowledge and skills, liaise with external health care services.
  • Utilise autonomy when managing operational factors that influence patient access and improve patient journey
  • Engage with all stakeholders in the initiation and development of internal and external strategies to inform improvements in patient journey including CEC, Whole of Health Program.

Selection Criteria

  1. Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA)
  2. Minimum five years full time equivalent post qualification experience, with at least three years full time equivalent experience in Care Coordination, case management, complex care needs and/or relevant post graduate qualifications.
  3. Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework.
  4. Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research.
  5. Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
  6. Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
  7. Demonstrated clinical expertise in client-centred consultancy with the ability to work independently.
