The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Make a meaningful difference by supporting people living with advanced, life-limiting illness. We are seeking a dedicated Speech Pathologist to join our Supportive and Palliative Care team, providing person-centred care to patients and their families throughout their care journey.
In this role, you will provide evidence-based speech pathology assessment and intervention to patients with advanced, life-limiting illness. Using a person-centred approach, you will work with patients and their families to understand individual goals, optimise communication and swallowing function, and support comfort and quality of life.
Working as part of the interdisciplinary Supportive and Palliative Care team, you will contribute to the delivery of coordinated care across inpatient and community settings, helping to ensure continuity of care throughout the patient journey. The role also requires a willingness to travel to other sites within the Local Health District as needed to support service delivery.

What you will bring to the role
- Bachelor’s or post graduate degree in speech pathology which provides eligibility for membership of Speech Pathology Australia, or other qualification deemed equivalent by the employer.
- Demonstrated ability to assess and manage communication and swallowing disorders in adults with advanced, life-limiting illness, chronic illness, oncology or palliative care needs.
- Demonstrated effective written and verbal communication skills.
- Demonstrated effective time management skills.
- Demonstrated commitment to professional development and demonstrated knowledge of quality improvement and evidence-based practice.
- Demonstrated experience working in multidisciplinary team settings, and ability to work autonomously with limited direct supervision.
- Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
