What you'll be doing
This position is responsible for the provision of a Social Work service to clients of Northern Sydney Home Nursing Service (NSHNS), and other Chronic Disease Management (CDM) services, residing within Northern Sydney LHD. This involves assessment and service provision, including counselling and referral to appropriate services for NSHNS and CDM clients referred to Social Work. The position requires excellent, timely communication with members of the NSHNS /CDM teams to ensure high quality case management, appropriate clinical handover and transfer of care.

Candidates will need to meet the following criteria:
- A Bachelor or post graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW). Experience at a Level 2 Social Work or equivalent level, and demonstrated skills in counselling/casework, with individuals and families, both short and long term. Ability to demonstrate advanced reasoning skills and operate autonomously with minimum direct clinical supervision.
- Provide evidence of proficient written, verbal and interpersonal communication skills with the proven ability to liaise with novice and experienced health professionals and external service providers. Demonstrated computer literacy and ability to use health information systems.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process. Experience in clinical teaching and supervision of students, level 1/2 Allied Health Professionals and/or Allied Health Assistants.
- Demonstrated proficient skills and knowledge in the provision of current social work practice. A strong commitment to an evidence-based service to provide for consumers, carers and families requiring assessment, case management, intervention, appropriate referral and discharge planning.
- Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement.
- Experience working in an acute hospital or community health setting; working with people with chronic disease, dealing with issues of loss, grief and bereavement; assessment, case management and discharge planning.
- NSW Drivers Licence.
