What you'll be doing
This position is responsiblie for providing high quality social work services, clinical leadership and care coordination as an integral member of the multidisciplinary healthcare team within the Transition Aged Care Program (TACP) and Aged Care Transitional Intervention Program (ACTIP) in the Hastings Macleay Clinical Network
The role is responsible for supporting older people with complex health and care needs to achieve optimal health outcomes following hospital discharge.
About You
You are a compassionate and experienced Social Worker with qualifications eligible for membership of the Australian Association of Social Workers and a strong commitment to supporting older people to achieve their goals, maintain independence, and transition successfully through stages of care. Through your work within aged care and community health settings, you provide person-centred support that addresses the complex social, emotional, health, and practical needs of individuals and their families.
You excel at working collaboratively within multidisciplinary teams, applying strong problem-solving skills to assess needs, coordinate services, and develop tailored care plans that support positive outcomes for clients participating in programs such as the Transition Aged Care Program (TACP) and Aged Care Transitional Interventional Program (ACTIP). Your ability to navigate complex situations and foster effective partnerships contributes to the delivery of seamless, high-quality care across the broader aged care service.
You are recognised for your excellent communication skills and your ability to engage effectively with clients, carers, healthcare professionals, and community providers. Through clear written, verbal, and electronic communication, you identify key issues, advocate for client needs, and ensure coordinated care that aligns with best practice and individual goals.
Committed to evidence-based practice, trauma-informed care, and continuous quality improvement, you actively contribute to service development and support excellence in care delivery. You are confident providing professional guidance, education, and clinical support to colleagues, including supervision and mentoring, to strengthen team capability and service outcomes. With a current NSW Driver Licence and the flexibility to travel as required, you are dedicated to delivering accessible and responsive services to older people within their communities.
