Location
Based within the newly established Newcastle Palliative Care Hub, this role offers a unique opportunity to deliver high-quality palliative and end-of-life care across hospital and community settings. You’ll be part of a supportive multidisciplinary team - nurses, allied health professionals, medical specialists, volunteers, and GPs - working together to ensure patients and families receive compassionate, person-centred care in the place that feels right for them.
About the Role
Social Work: In this newly created role, you will provide evidence-based palliative and psycho-social support to patients, families, and carers living with progressive, life-limiting conditions. You’ll work collaboratively across home and hospital environments, helping people identify and achieve their goals of care. In this role you will utilise a hybrid model of face-to-face and virtual care to ensure equitable access and exceptional patient and family outcomes. You are an experienced Social Worker with strong interpersonal skills, emotional resilience, and a commitment to person-centred, end-of-life care. You thrive in a collaborative multidisciplinary team, confidently prioritise a dynamic caseload, and bring a commitment to improving care and service delivery through innovation. You enjoy sharing knowledge with colleagues and students and contribute positively to a culture of learning, teamwork, and continuous improvement.
Requirements
Must hold as a minimum a bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
How We Will Support You
You will receive comprehensive orientation and ongoing guidance from the Director of Allied Health for Integrated Care along with support from the multidisciplinary team and robust professional practice supervision. You’ll be part of a broader network of palliative care clinicians across the District connected by the Palliative and End of Life Care Services Stream. Your manager will provide regular check-ins, development opportunities, and annual reviews to support your wellbeing and continued professional growth.
