About the Role
As a Social Worker within our Inpatient Rehabilitation, Stroke and Critical Care teams, you will play a vital role in delivering patient-centred care across a diverse and rewarding clinical environment. Working collaboratively with medical, nursing and allied health professionals, you will provide comprehensive psychosocial assessments, intervention planning, patient advocacy and discharge coordination to support positive health outcomes.
This role offers the opportunity to work with patients and families navigating complex health challenges, facilitating family meetings, coordinating care plans and connecting individuals with appropriate support services. You will contribute your expertise within a collaborative multidisciplinary setting while managing a varied caseload and making a genuine impact on the wellbeing of patients and their loved ones.
About You
You are a compassionate and skilled Social Worker who thrives in a dynamic healthcare setting. With a strong understanding of the social work role within acute and rehabilitation services, you are confident in conducting psychosocial assessments, developing effective intervention strategies and supporting patients through complex care pathways.
You value teamwork and enjoy working alongside a range of healthcare professionals to achieve the best possible outcomes for patients. Highly organised and adaptable, you are able to manage competing priorities while maintaining exceptional communication and documentation standards. Your ability to build rapport, facilitate family discussions and coordinate complex case management plans ensures patients and families receive the support they need during challenging times.
To be successful in this role, you will hold a degree qualification that provides eligibility for membership with the Australian Association of Social Workers (AASW) and demonstrate a commitment to delivering high-quality, person-centred care.
What you'll be doing
To provide an effective and efficient Social Work service that addresses the psychosocial aspects of care for patient, as identified.
To provide intervention including support, education and resources to people in the context of their bio-psycho-social situation, to assist in dealing with health events.
