Blue Mountains District ANZAC Memorial Hospital is an acute hospital that supports a population of approximately 80,000 and is the only ANZAC Memorial hospital remaining in NSW. Easily accessible to Sydney residents, our team of clinicians, allied health, and support staff works with the community on a shared goal to maintain the delivery of high-quality healthcare in the Blue Mountains region.
The social worker accepts referrals from medical, nursing and allied health staff and from patients and their families and initiates referrals where appropriate.
We are seeking a compassionate and motivated Social Worker to join our multidisciplinary team and contribute to high‑quality, patient‑centred care. In this role, you will work closely with patients, families, and health professionals to address the emotional and social challenges that arise from illness, disability, hospitalisation, and significant changes in life circumstances.
As a key member of the healthcare team, you will respond to referrals from medical, nursing and allied health staff, as well as from patients and their families. You will also identify and initiate referrals when appropriate. Using a range of social work methods, you will undertake comprehensive psychosocial assessments, develop and support management and discharge planning in collaboration with other clinicians, and provide continuing social work services tailored to the needs of each patient and their family.
Your work will involve providing crisis intervention, therapeutic counselling, grief and bereavement support, and the coordination of community and social welfare resources. You will assist with arranging alternative accommodation when required, ensure appropriate follow‑up services are in place after discharge, and actively participate in family conferences, interviews, case discussions and team meetings to ensure a cohesive and supportive approach to care.

What you will bring to the role
- Tertiary Qualifications in Social Work and eligibility for the Australian Association of Social Workers (AASW)
- Demonstrated experience in the provision of a range of social work services and experience working in a hospital setting.
- Experience in working with clients with chronic and complex needs resulting from disability and illness including need for Aged Care Placement, Guardianship and NDIS applications
- Experience / interest in working in the area of Women’s and Children’s Health including supporting patients with their pregnancy journey including safety and risk assessments and pregnancy loss.
- Experience in providing psychosocial assessment, crisis counselling, grief and loss counselling and conflict resolution
- Demonstrated ability to provide high quality professional assessment and care plans within a fast paced setting where there are competing demands
- Demonstrated high level communication skills, (written and oral) negotiation advocacy and demonstrated computer skills
- Current Drivers Licence, including P1 or above, valid within Australia
