Where you'll be working

Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
 
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
 
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

This is an Aboriginal Identified role - we need someone to bring their cultural capability and lived experience as an Aboriginal person to this role. What you'll be doing

The purpose of the role of the Aboriginal Health Outreach Worker (Integrated Team Care Program) is to: 

  • Establish links with local Aboriginal and Torres Strait Islander peoples to encourage and support the increased use of health services within SESLHD.
  • Understand and identify barriers and solutions for local Aboriginal and Torres Strait Islander peoples in accessing health services in SESLHD.
  • Support health professionals and health services to ensure culturally appropriate and safe policies and practices are implemented when engaging with Aboriginal and Torres Strait Islander peoples.

    Selection Criteria

    1. To be a person of Aboriginal or Torres Strait Islander descent. Aboriginal / Torres Strait Islander is a genuine occupational qualification and is authorised under Section 14(d) of the Anti-discrimination Act 19.
    2. Holds or aspires to hold a minimum III health qualification in the area of care in which the Aboriginal Health Workers works and relevant experience in the delivery of Aboriginal Health Programs.
    3. Demonstrated highly developed skills in oral and written communication and computer literacy.
    4. Demonstrated ability to inform and support the development of Aboriginal health programs/services and their implementation.
    5. Ability to engage with Aboriginal organisations, mainstream health services, NGO's and government services.
    6. Comprehensive understanding of challenges and barriers related to Aboriginal health, particularly around chronic disease.
    7. Demonstrated experience in case management or care coordination working with Aboriginal families.
