What you'll be doing
SESLHD’s Sexual Health and Blood Borne Virus Services (SHBBV) provides leadership, innovation and research in the sector. Services within SHBBV are involved in the prevention, diagnosis, and treatment of sexually transmissible infections (STIs) and blood borne viruses, as well as the health and wellbeing of those at risk of such infections.Across these services, which include both SESLHD and statewide teams, care is provided via fixed locations and outreach networks to a client base of approximately 28,000 individuals.SHBBV has highly skilled and diverse staff including nursing, medical, allied health, health promotion, administrative and technical staff. It is the largest SHBBV service in Australia.
The Statewide Clinical Support Services, within Sexual Health and Blood Borne Virus Services, comprises a range of both statewide and local teams that deliver a broad spectrum of care. These services include case management, psychology, nutrition, social support, contact tracing, and gender-affirming care. The teams are committed to working collaboratively to provide a coordinated, integrated, and seamless service experience for clients.
The Clinical Outreach team within the Statewide Clinical Support Service, has 2 streams the HIV Outreach Team and Adahps. The HIV Outreach Team (HOT) provides case management for people living with HIV (PLHIV) with complex health and social needs, focusing on those with co occurring mental health and substance use issues. Services include psychosocial assessments, counselling, and targeted health education to improve health outcomes and reduce care access inequalities. The work is conducted in a range of contexts including home visits, outreach work and some center based activities.

Selection Criteria
- Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia.
- Demonstrated understanding in the clinical management of clients with complex care health issues, with a commitment to the principles of case management.
- Knowledge of care and treatment issues related to HIV and other comorbidities.
- Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
- Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Basic computer skills and proven ability to use health information systems.
- A commitment to quality improvement, practice development, evidence based practice, education and research to achieve quality outcomes for clients.
- Current unrestricted NSW Driver’s License.
