An opportunity has become available to join the Social Work team within our Virtual Aged Care Services (VACS). The VACS team often provides service within patient’s homes and other non- acute health settings such as residential aged care facilities. One of the key services is providing social work interventions as part of a multidisciplinary team to both inpatients and outpatients.
As the Social Worker, you will be required to complete comprehensive psychosocial assessments and provide varying types of counselling. The successful incumbent will facilitate family/provider meetings and contribute to case planning/ management in conjunction with the multi-disciplinary team. What you will bring to the role
- A Social Work Degree accredited by the Australian Association of Social Workers
- Proven experience working in Aged Care and extensive knowledge around concerns and psychosocial stressors for people over 65 years of age. Experience and knowledge of mandatory reporting requirements.
- Proven experience in crisis intervention, grief and loss counselling, end of life concerns, identifying and responding to psychosocial stressors and carer stress. Strong skills in mediation and conflict management, particularly in relation to complex family dynamics.
- Demonstrated ability to work independently with minimal supervision and to make clinical decisions within a short timeframe. Proven ability in applying sound professional knowledge and clinical judgement when performing novel, complex or critical tasks. Experience in making applications to the NSW Civil and Administrative Tribunal (NCAT).
- Proven commitment to professional development and supervision. Ability to provide supervision to junior staff.
- Demonstrated ability to work as a member of a team; basic computer skills; proven time management skills; excellent oral and written communication skills
- Current NSW drivers license willingness to use for work purposes. This may include driving long distances across the Local Health District.
