About the role
Join a team of passionate Social Workers providing services across Lachlan Health Service. The caseload for this role is rural generalist including in patient, out patients, adults and children.

The successful applicant must be eligible for membership with the AASW and have a minimum of 3 years post graduate experience working in health and/or human services. The position provides generalist social work services to a diverse range of community and hospital clients (cradle to grave) with priority given to those most at risk, although social work is not a crisis service.

The position will demand comprehensive skills in multi-dimensional assessment using bio-psychosocial assessment, a trauma informed, person centred and recovery-based approach to clients, their families and significant others. As a health-based position the position works with a wide range of other allied health disciplines, medical, nursing staff and external agencies forming a multi-disciplinary approach toward best client outcomes. There are presently seven social workers employed across Lachlan who meet regularly to provide mutual support and promote ongoing education.

With Western local health district, the area social workers meet twice yearly for networking and educational purposes. This temporary position is to back fill the Forbes single FTE with another level 3 social worker working 2 days per week. The work is primarily community Health ambulant care clients with some support to complex inpatient services provided on an as available basis. Social workers assist their clients in areas of problem solving and promoting physical and psychological well-being.

They work across areas of: mental health, child protection, social justice, human rights advocacy, community development, counselling and psycho-education. Within this role there is scope for self- direction and autonomy when using evidenced based approaches to work successfully with clients. Supervision is provided monthly.
