The Social Work Senior Clinician Liver and Cardiology Supportive Care provides high level Social Work service to patients, carers and families to ensure the provision of optimal health outcomes consistent with the districts and professional policies, procedures, standards, relevant legislation and Code of Conduct.

- A completed bachelor’s degree in social work that provides eligibility for membership of the Australian Association of Social Workers with demonstrated experience as a Level 2 or equivalent.
- Demonstrated extensive clinical skills and knowledge in comprehensive bio-psychosocial assessments, counselling and therapeutic interventions, complex clinical casework/case management, advocacy and service coordination.
- Comprehensive understanding and demonstrated social work experience in the social and emotional needs of patients living with late stage liver disease or cardiac failure.
- Demonstrated experience in clinical and team leadership including proven capacity to teach and supervise Level 1/2 Allied Health Professionals, Allied Health Assistants and students.
- Demonstrated excellent interpersonal, written and verbal communication, negotiation, liaison, advocacy, conflict resolution skills, with computer proficiency and proven ability to work collaboratively in a multidisciplinary team and with key stakeholders.
- Demonstrated and high-level prioritisation and reasoning skills with proven ability to work autonomously under limited supervision, manage a demanding workload, apply reasoning skills and exercise independent professional judgement.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation processes.
