About the Role 

Sydney Local Health District is expanding its community mental health service to better service our community with the creation of new Senior Mental Health Clinician roles across our care coordination teams. There are five (5) permanent full time roles available at either Allied Health (Levels 3 or 4) or Clinical Nurse Consultant (Grade 1 or 2).

This is an exciting opportunity for experienced mental health professionals to step into a clinical leadership role within community-based mental health services. 

 

Our Community Mental Health Services are driven by excellence and a commitment to provision of the best possible care, delivering care across our five community sites at Marrickville, Redfern, Camperdown, Canterbury and Croydon.

These roles will be embedded in the care coordination teams - the mainstay of the community mental health services for medium to long term care.

The Senior Mental Health Clinicians will:

·         Support and guide a multidisciplinary team in delivering accessible mental health support to consumers who are experiencing enduring mental health care needs.

·         Support junior clinicians in decision making for complex cases

·         Partner with primary care and non-government organisations to transition care of consumers

 

This is the first time such a position has been offered, and the successful applicant will be responsible for the development and implementation of this clinical pathway within community mental health services and Core Teams. So come and join our team!

Ideal Candidate

The successful candidate will;

  • Hold current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience.
  • Provide clinical leadership and support a skilled team of mental health professionals in promoting workflow within Community Core Teams
  • Provide clinical oversight, support, and guidance on complex presentations to the services
  • Provide support to Team Leaders and clinicians in identifying discharge pathways for care coordinated consumers
  • Build strong relationships with key internal and external health agencies and community support services to support timely and effective transition of care
