About the role
If you are an experienced Dietitian passionate about working with the elderly to achieve their health care outcomes, join the Transitional Aged Care Program (TACP)!
Your day to day will include assessment and management of malnutrition risk, weight loss, frailty, diabetes, wounds, texture-modified diets and nutrition support. Working with a multi-disciplinary Allied Health team, you will be encouraged to expand your clinical skills and provide a service with a focus on re-enablement and person-centred care.
This is a truly rewarding experience, where you can be a part of someone’s life, not just their illness. The TACP report that the most rewarding part of this role is supporting older people to regain confidence, improve their function and remain safely at home after a hospital admission.
