About the Role 

The Healthy at Home Program supports vulnerable clients through comprehensive assessments and tailored home-based care, aiming to prevent hospitalisation and support safe discharge from acute care settings. As a Registered Nurse in this team, your responsibilities will include:
  • Conducting comprehensive nursing assessments in the community
  • Participating in geriatric assessments and clinical observations
  • Providing wound care, pathology collection, and clinical reviews
  • Liaising closely with Geriatricians, GPs, medical officers, and other health and community services
  • Supporting short-term, coordinated care packages in collaboration with a multidisciplinary team

This role is part of a broader NSW Health initiative, integrated into the Out of Hospital Care (OHC) framework and Community Packages (ComPacks) scheme, delivering up to six weeks of intensive in-home support post-hospital discharge.
