The Nepean Mental Health Centre provides inpatient care for people experiencing acute mental health issues through the short-stay Psychiatric Emergency Care Centre (PECC), the Acute Mental Health Unit, Mental Health High Dependency Unit, and Older Persons Mental Health Unit. We support our Patients by linking them to case managers for ongoing treatment and support within an environment appropriate to the person's needs.
We are seeking a compassionate and motivated Registered Nurse to join the Mental Health Intake and Referral Team (I&R) within Nepean Blue Mountains Local Health District (NBMLHD). In this role, you will work collaboratively with patients, families, carers, and multidisciplinary teams to deliver safe, person-centred care across community mental health services. As an Intake and Referral Clinician, you will be responsible for reviewing referrals received via the Patient Flow Portal from Health Direct and coordinating timely access to appropriate services for individuals referred for community-based mental health care.
You will play a key role in assessing referral information, facilitating care pathways, and ensuring consumers are linked to the most suitable mental health supports within the district.Positioned at the front door of Community Mental Health access, you will work closely with the Triage and Assessment Centre (TAC) to support individuals presenting to Emergency Departments or TAC who require ongoing community care. The role involves conducting warm handovers to receiving teams, supporting continuity of care, and completing post-discharge follow-up for consumers transitioning from inpatient settings to the community.

What you will bring to the role
- Current registration as a Registered Nurse Division 1 with AHPRA.
- Demonstrated experience and clinical skill in nursing as a Registered Nurse including completion of a new graduate Registered Nurse program or equivalent experience working as a Registered Nurse.
- Demonstrated capacity to provide effective nursing care that is person centered, family inclusive and culturally sensitive to people accessing health services.
- Demonstrated ability to organise and prioritise work.
- Demonstrated effective communications written and verbal skills.
- Demonstrated commitment to professional development.
- Ability to work effectively within a multi-disciplinary team.
