Project Officer, Youth Transition Care

Essential Criteria:

Vaccination Category: B
Checks: National Criminal Record Check and Working with Children Check
License: Drivers License valid in NSW
Qualification: Bachelor level qualification in a relevant health discipline and relevant experience.

Selection Criteria:

1. Demonstrated successful track record in project management including change management and implementation methodology, reporting, monitoring and evaluation

2. Demonstrated knowledge and understanding of the Transition of Care of young people with chronic conditions in WSLHD and SCHN

3. Demonstrated ability to collaborate and work in partnership with multiple stakeholders, both internal and external to the organisation

4. Demonstrated time management skills

5. Demonstrated high level communication skills, written and verbal, negotiation, report writing and research skills
