Where you'll be working:
This full-time contract is with the Transitional Aged Care Program (TACP), a Commonwealth-funded service supporting older people as they transition safely from hospital back into the community. Designed for those who would typically require residential aged care but have the potential to regain independence, the program runs for up to 12 weeks and focuses on improving physical, cognitive, and emotional function.
Based in Taree, you'll join a collaborative, multidisciplinary team delivering care across the Lower Mid North Coast. You'll work alongside a passionate group including Registered Nurses, Physiotherapists, Occupational Therapists, Domestic Support Workers, ACAT Assessors, Dementia Support, Psychogeriatric Services, and Allied Health professionals—all part of a broader Aged Care Services team committed to delivering excellence.

About the role:
As the TACP Physiotherapist, you’ll play a vital role in helping clients reach their goals and return safely to independent living. Working closely with the multidisciplinary team, you’ll provide physiotherapy assessments, interventions and support to clients in their homes as they transition from hospital. The position includes onsite training and robust support from a network of clinical and non-clinical professionals, including the NUM ACAT & TACP, Aged Care Service Manager, Psychogeriatric Team, and administrative staff. You’re a dedicated, personable professional who takes pride in delivering exceptional patient-centred care. Your strong communication skills and ability to work independently in a community setting are essential. You are passionate about working with older adults and committed to supporting them in achieving their goals and maintaining independence at home in accordance with TACP guidelines.
Requirements:
- Tertiary qualifications in Physiotherapy
- Current registration with AHPRA
- Demonstrated ability to provide physiotherapy in a community or aged care setting
- Excellent interpersonal and communication skills
- Current NSW Driver Licence and willingness to travel for community visits
- Commitment to ongoing professional development
