About the role
In this role, you will conduct physical, mobility, and fall-risk evaluations, deliver targeted exercise programs, strength training, pain management, source and prescribe relevant mobility aids and equipment in community-based settings under the guidance and support of a Senior TCP Physiotherapist.
The role requires excellent communication, documentation, prioritisation and time-management skills, with the ability to work independently while contributing to a multidisciplinary, goal-focused reablement program.
TCP is a supportive multidisciplinary team made up of Clinical Care Coordinators and Allied Health clinicians, including occupational therapy, physiotherapy, social work, speech pathology and dietetics. You will be part of a district-wide service that works collaboratively across community TCP settings.
This is a truly rewarding role, where you can support older people to regain confidence, increase their quality of life and remain safely at home after a hospital admission.
The successful applicant will be supported through orientation to TCP processes, referral pathways, documentation systems and clinical governance requirements. Ongoing support will be provided through the Allied Health Team Leader, clinical supervision, multidisciplinary case conferences, peer support and opportunities for professional development.
