About Your Role

The Bereavement Support social worker/Psychologist will be the clinical lead for the Bereavement Support in the Community service.
This service will provide:

  • Coordinated support and bereavement follow up of family members/carers of patients of the MLHD Palliative Care Service.
  • Consistent provision of information and psycho-education for bereaved family members/carers of patients of the MLHD Palliative Care Service
  • Bereaved family members/carers have awareness of the available support services and resources, including the availability of bereavement counselling through MLHD for family members/carers of patients of the MLHD Palliative Care Service who meet referral criteria.


The role will be part of the MLHD Community Care team and work within the Palliative Care team and closely with the Community Care Nursing team to identify and link clients who are receiving or who have received Palliative Care services and require bereavement support.

Bereavement counselling seeks to ’restore functioning, facilitate wellbeing and reaffirm or reconstruct meaning for at risk bereaved people’ (Stroebe, Schut and Stroebe 2005 in Klasen, 2017: 363).

The Bereavement Counsellor offers individual counselling for bereaved family members, caregivers and friends of supportive and palliative care patients who have died and were known to MLHD. The Bereavement Counsellor also coordinates a range of other forms of support (eg support groups, bereavement information and resources, memorial services, referral for other specialist intervention or support as appropriate) where need is identified, as well as the provision of education and consultation for MLHD staff and the community regarding bereavement related issues.
